**68 Ga-IMP-288** is a peptide-based diagnostic radiopharmaceutical used in pretargeted immuno-positron emission tomography (immuno-PET) imaging for cancer detection. It consists of a di-Histamine-Succinyl-Glycine (di-HSG) hapten peptide chelated with DOTA and labeled with the positron-emitting radionuclide gallium-68. The compound is administered following a bispecific monoclonal antibody (BsMAb, such as TF2), which first targets carcinoembryonic antigen (CEA) on tumor cells and presents a second specificity for the HSG hapten. Upon administration, 68 Ga-IMP-288 binds to the BsMAb at tumor sites, enabling high-sensitivity PET imaging with high tumor-to-background ratios. Primary indications include imaging of metastatic colorectal cancer, medullary thyroid carcinoma, and HER2-negative breast cancer expressing CEA. The technology has shown promising safety and diagnostic performance in early clinical trials and aims to improve specificity and sensitivity over conventional imaging agents by using a pretargeting approach[1][2][3][4][6].
