68Ga-ACN376 is a gallium-68 labeled radiopharmaceutical imaging agent developed by Peking University Cancer Hospital & Institute. It consists of a single-domain antibody (nanobody), ACN376, which specifically targets the claudin-18 isoform 2 (CLDN18.2) protein. CLDN18.2 is a tight junction protein frequently overexpressed in various solid tumors, particularly gastric, esophageal, and pancreatic cancers, while remaining largely absent in healthy tissues. The nanobody is conjugated to the chelator NOTA and labeled with the positron-emitting isotope gallium-68 for use in PET/CT imaging. This agent is designed to non-invasively assess CLDN18.2 expression levels in vivo, serving as a companion diagnostic to identify patients likely to benefit from CLDN18.2-targeted therapies. Due to its small molecular size (approximately 15 kDa), 68Ga-ACN376 exhibits rapid blood clearance and high tumor penetration, allowing for same-day imaging.
