68Ga-DOTA-IBA is a novel radiopharmaceutical agent consisting of gallium‑68 labeled DOTA‑ibandronic acid, designed primarily for positron emission tomography/computed tomography (PET/CT) imaging of bone metastases. As a bisphosphonate-based compound, it exhibits high affinity for bone tissue, particularly at sites of increased osteoblastic activity such as metastatic lesions. The mechanism involves targeting areas with active bone remodeling by binding to hydroxyapatite in the bone matrix. This enables sensitive and specific detection of skeletal metastases in various cancers, including breast cancer and Ewing sarcoma. Preclinical and early clinical studies indicate rapid blood clearance, strong bone uptake, low toxicity, and favorable safety profile. The agent is also being explored as part of a theranostic pair with therapeutic isotopes like lutetium‑177 (177Lu), offering potential for both diagnosis and therapy monitoring in patients with malignant tumors involving the skeleton[2][3][4][5][6][7][8][9][10].
