68Ga-DOTA-NI-FAPI04 is a novel radiopharmaceutical agent designed for positron emission tomography (PET) imaging. It targets fibroblast activation protein (FAP), which is highly expressed in cancer-associated fibroblasts (CAFs) within the tumor microenvironment and in fibrotic tissues. The compound incorporates a hypoxia-sensitive nitroimidazole moiety into the FAP-targeting agent FAPI-04, resulting in enhanced tumor accumulation and retention compared to earlier FAP tracers. This dual-targeting approach allows for improved imaging of both FAP expression and hypoxic regions within tumors or fibrotic tissue. Preclinical studies have demonstrated strong binding affinity to FAP and superior uptake/retention in tumor models relative to standard 68Ga-labeled FAPI agents. Clinically, it is being investigated for diagnostic imaging across various solid tumors as well as liver fibrosis[1][4][6][10].
