68Ga-DOTANOC is a diagnostic radiopharmaceutical imaging agent used in positron emission tomography (PET) combined with computed tomography (CT) for the detection, staging, and management of gastroenteropancreatic neuroendocrine tumors (GEP-NETs). It consists of the positron-emitting radionuclide gallium-68 complexed with DOTANOC (1,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid-1-Nal3-octreotide), a somatostatin analog. The agent functions by binding with high affinity to somatostatin receptors (SSTR), particularly subtypes SSTR2, SSTR3, and SSTR5, which are frequently overexpressed on the surface of neuroendocrine tumor cells. This binding allows for high-resolution visualization of primary tumors and metastatic lesions. In clinical research led by Assistance Publique Hopitaux De Marseille (the GEP-NOC study), it is being evaluated for its impact on clinical decision-making and patient management compared to standard imaging techniques like somatostatin receptor scintigraphy (SRS).
