68Ga-FAPI radiotracer is a gallium-68 labeled small-molecule radiopharmaceutical designed to target fibroblast activation protein (FAP). FAP is a cell-surface serine protease that is highly expressed on cancer-associated fibroblasts (CAFs) in the stroma of many epithelial tumors, while showing low expression in normal tissues. This differential expression makes FAP an attractive target for diagnostic imaging via positron emission tomography (PET). The tracer is being investigated for its ability to detect various malignancies, including peritoneal metastases, where it may offer superior sensitivity compared to standard [18F]FDG PET/CT due to high stromal FAP expression and low background activity in the abdomen. The INTERACT-FAPI Phase 1 study at Erasmus MC is specifically evaluating its utility in patients with peritoneal carcinomatosis.
