68Ga-GSI is a radiopharmaceutical designed as a positron emission tomography/computed tomography (PET/CT) probe. It targets Granzyme B (GzmB), a protein involved in the immune response. This diagnostic agent is being developed to evaluate early tumor responses to immunotherapy, particularly immune checkpoint inhibitors, by reflecting the immune activation state within tumors. It is currently under clinical investigation for various cancers, including gastric cancer, esophageal cancer, and non-small cell lung cancer.
