68Ga-HBED-CC-WL12 is a radiopharmaceutical designed for positron emission tomography (PET) imaging of programmed death-ligand 1 (PD-L1) expression. It consists of the small peptide WL12, which acts as a high-affinity PD-L1 antagonist, conjugated to the chelator HBED-CC and labeled with the positron emitter Gallium-68 (68Ga). By non-invasively mapping PD-L1 levels in the tumor microenvironment, this agent aims to identify patients likely to respond to PD-L1/PD-1 blockade therapies and monitor treatment response in real-time. Preclinical studies have demonstrated its specificity in PD-L1-positive cell lines and its potential in imaging pancreatic cancer and other malignancies, showing favorable pharmacokinetics and image quality compared to other tracers like 18F-FDG or 68Ga-FAPI-04.
