68Ga-NB381 is a novel diagnostic radiopharmaceutical composed of a nanobody targeting CD38, labeled with the positron-emitting radionuclide gallium-68 (68Ga). It is designed as a PET/CT imaging probe for the detection and evaluation of malignancies that overexpress CD38, most notably multiple myeloma. The nanobody component provides high affinity and specificity for CD38, enabling sensitive imaging of malignant plasma cells. The agent is conjugated with gallium-68 using TOHP as a bifunctional chelator, resulting in high in-vivo stability and significant tumor accumulation in preclinical models. Its primary clinical application is to improve early diagnosis, disease monitoring, and treatment planning for multiple myeloma by providing non-invasive visualization of CD38 expression levels. This can also support assessment of therapeutic response to anti-CD38 therapies such as daratumumab[1][2][5][6].
