68Ga-NOTA-RM26 is a radiopharmaceutical diagnostic agent composed of RM26, a synthetic bombesin analogue and antagonist of the gastrin-releasing peptide receptor (GRPR), conjugated via the macrocyclic chelator NOTA (1,4,7-triazacyclononane-N,N',N''-triacetic acid) to the positron-emitting radionuclide gallium-68. It is designed for use in positron emission tomography/computed tomography (PET/CT) imaging to non-invasively detect and characterize tumors that overexpress GRPR. The primary mechanism involves selective binding to GRPR on tumor cells, enabling visualization of receptor expression in vivo. This agent has been investigated primarily for imaging high-grade gliomas and prostate cancer, with studies demonstrating its safety and efficacy in distinguishing tumor grades and guiding biopsies[1][3][6][8][9].
