68Ga-PFD3 is a gallium-68 labeled radiopharmaceutical diagnostic agent consisting of a nanobody (single-domain antibody) specific for Delta-like ligand 3 (DLL3). DLL3 is a cell-surface protein that is rarely expressed in healthy adult tissues but is significantly overexpressed in over 80% of small cell lung cancers (SCLC) and other high-grade neuroendocrine tumors (NETs), making it a highly specific target for molecular imaging. The nanobody platform (~15 kDa) offers distinct advantages over conventional monoclonal antibody-based tracers, including superior tissue penetration, rapid blood clearance, and a shorter in vivo residence time. These properties allow for high-contrast PET/CT imaging within hours of injection. 68Ga-PFD3 is currently being evaluated in Phase II clinical trials to assess its diagnostic performance and clinical impact in patients with metastatic neuroendocrine neoplasms compared to standard imaging modalities like FDG or PSMA PET/CT.
