Overview

This is a combination of two radiopharmaceutical agents used in the management of prostate cancer: **1. [^68Ga]Ga‑PSMA‑11:** A gallium‑68–labeled small molecule that binds to prostate-specific membrane antigen (PSMA), which is highly expressed on most prostate cancer cells. It is administered intravenously and used as a positron emission tomography (PET) imaging agent to localize and stage PSMA-positive lesions in men with suspected or recurrent prostate cancer. The mechanism involves specific binding to the extracellular domain of PSMA, allowing visualization of tumor burden via PET[1][2][5][6]. **2. [^177Lu]Lu‑PSMA‑617:** While not part of this combination by itself, ^177Lu-labeled PSMA‑617 is a therapeutic radioligand for metastatic castration-resistant prostate cancer (mCRPC). However, ^68Ga-labeled PSMA‑617 can be used as an investigational diagnostic PET tracer similar to ^68Ga‑PSMA‑11. **Combination Use:** The combination "68Ga‐PSMA‐11 + 68Ga‐PSMA‐617" refers to using both tracers for diagnostic purposes—primarily for selecting patients eligible for subsequent targeted radioligand therapy with ^177Lu– or other radionuclide-labeled versions—by confirming sufficient tumor uptake on PET/CT scans[3][4][7][8]. Both agents target the same molecular marker but may have slightly different pharmacokinetics or availability. Mechanistically, both are small-molecule ligands that bind specifically and with high affinity to the extracellular domain of human PSMA; after intravenous administration, they accumulate in tissues expressing high levels of this antigen and are visualized by PET due to their positron-emitting radioisotope (^68Gallium)[1][5].
