68Ga-SC691 is a gallium-68 labeled small molecule radiopharmaceutical designed to target prostate-specific membrane antigen (PSMA), also known as glutamate carboxypeptidase II. Developed primarily for the diagnostic imaging of prostate cancer, it utilizes a urea-based targeting motif optimized with a specific linker and bifunctional chelator to enhance binding affinity and internalization into tumor cells. In positron emission tomography (PET) imaging, 68Ga-SC691 allows for the sensitive detection of primary prostate cancer lesions and metastatic disease, including in patients with low prostate-specific antigen (PSA) levels during biochemical recurrence. Preclinical and early clinical evaluations suggest it offers high tumor-to-background ratios and comparable or superior imaging quality to established tracers like 68Ga-PSMA-11.
