68Ga-TROP2 is an investigational radiopharmaceutical PET tracer designed for the non-invasive detection and quantification of Trophoblast cell-surface antigen 2 (TROP2) expression in solid tumors. TROP2 is a transmembrane glycoprotein significantly overexpressed in various epithelial malignancies, including breast, lung, and gastric cancers, where it promotes tumor proliferation and metastasis. The tracer typically utilizes a high-affinity TROP2-targeting moiety, such as the nanobody Nb1159 or an antibody fragment, which is conjugated to a chelator (e.g., DOTA) and labeled with the positron-emitting radioisotope Gallium-68. By binding specifically to the extracellular domain of TROP2, 68Ga-TROP2 enables clinicians to visualize tumor lesions, assess TROP2 density, and potentially select patients who are most likely to benefit from TROP2-targeted therapies, such as antibody-drug conjugates (ADCs) like sacituzumab govitecan or datopotamab deruxtecan.
