Overview

6EH³R³Z is an experimental high-dose tuberculosis retreatment regimen developed by the Institute of Tropical Medicine (ITM) in Antwerp, Belgium. It consists of a 6-month course of ethambutol (E), pyrazinamide (Z), and triple-dose versions of isoniazid (H³; 15 mg/kg) and rifampicin (R³; 30 mg/kg). The regimen is specifically designed to treat recurrent rifampicin-susceptible pulmonary tuberculosis, including cases of relapse and treatment failure. By utilizing significantly higher doses of rifampicin and isoniazid than the standard-of-care, the regimen aims to overcome low-level drug resistance and improve sterilization of the infection. It is currently being evaluated in the Phase 3 Tri-Do-Re clinical trial (NCT04260477) to determine its safety and non-inferiority compared to the standard 6EHRZ regimen.
