7-hydroxymitragynine (7-OH) is a terpenoid indole alkaloid and a potent active metabolite of mitragynine, the primary alkaloid found in the kratom plant (*Mitragyna speciosa*). It acts as a potent agonist at the mu-opioid receptor (MOR) and is estimated to be significantly more potent than morphine. While naturally occurring in trace amounts in kratom leaves, it is often produced semi-synthetically from mitragynine for use in concentrated products such as tablets and gummies. 7-OH is primarily used for its analgesic properties but is associated with a high potential for abuse, addiction, and respiratory depression. Currently, it has no FDA-approved medical uses and is subject to significant regulatory scrutiny, with recommendations for Schedule I classification due to safety concerns.
