765IGF-MTX is an experimental protein-drug conjugate developed by IGF Oncology, LLC, designed to treat advanced hematologic malignancies. The drug consists of insulin-like growth factor (IGF) covalently linked to the cytotoxic antimetabolite methotrexate (MTX). Its mechanism of action relies on the overexpression of the insulin-like growth factor 1 receptor (IGF-1R) on the surface of malignant cells in conditions such as myelodysplastic syndromes (MDS), chronic myelomonocytic leukemia (CMML), and acute myeloid leukemia (AML). By using IGF as a targeting ligand, the conjugate binds to IGF-1R and is internalized via receptor-mediated endocytosis, delivering the methotrexate payload directly into the cytoplasm. Once inside, methotrexate inhibits dihydrofolate reductase, disrupting folate metabolism and DNA synthesis, leading to cell death. This targeted approach aims to increase the therapeutic concentration of methotrexate within cancer cells while reducing systemic exposure and associated toxicities.
