765IGF-MTX is a novel biologic drug conjugate (BDC) composed of an engineered variant of insulin-like growth factor-1, known as long-R3-IGF-1, covalently linked to the antifolate drug methotrexate. Developed by IGF Oncology, LLC, the conjugate typically carries 4 to 8 methotrexate molecules per protein. It is designed to selectively target malignant cells that overexpress the insulin-like growth factor type 1 receptor (IGF-1R), a common feature in various hematologic and solid tumors. Upon binding to IGF-1R, the conjugate is internalized via receptor-mediated endocytosis, delivering the cytotoxic methotrexate payload directly into the cell. This targeted approach aims to enhance the therapeutic index of methotrexate by increasing its concentration within tumor cells while reducing systemic exposure and associated toxicities. The drug has been evaluated in Phase I clinical trials for patients with high-grade myelodysplastic syndromes and acute myeloid leukemia.
