8-MOP + UVA, commonly known as PUVA (psoralen plus ultraviolet A) photochemotherapy, is a treatment regimen combining the photosensitizing agent 8-methoxypsoralen (methoxsalen) with long-wave ultraviolet A radiation. 8-MOP is a small molecule that intercalates into the DNA of cells; upon exposure to UVA light, it becomes chemically active and forms covalent cross-links between DNA strands. This process inhibits DNA synthesis and cell division, particularly in rapidly proliferating malignant T-cells, and induces apoptosis. The Madrilenian Group of Cutaneous Lymphomas has investigated this regimen, specifically comparing PUVA monotherapy to PUVA combined with interferon alpha-2a, as a first-line skin-directed treatment for early-stage mycosis fungoides (stages Ia, Ib, and IIa).
