89Zr-ACT-101 is a radiopharmaceutical diagnostic imaging agent being developed by Alpha Cancer Technologies for the detection and monitoring of various cancers. The agent consists of ACT-101, a recombinant, non-glycosylated form of human alpha-fetoprotein (AFP), which is site-specifically labeled with the positron-emitting radioisotope Zirconium-89 (89Zr). ACT-101 serves as a targeting moiety that binds with high affinity to the AFP receptor (AFPR). This receptor is highly expressed on the surface of a broad spectrum of solid and liquid tumor cells, as well as myeloid-derived suppressor cells (MDSCs) within the tumor microenvironment, but is generally absent on healthy adult tissues. By utilizing positron emission tomography (PET) imaging, 89Zr-ACT-101 enables the non-invasive visualization of AFPR-positive tumors, potentially serving as a companion diagnostic to identify patients who may benefit from AFP-targeted therapeutic interventions, such as 177Lu-ACT-101 or AFP-drug conjugates.
