89Zr-miltuximab is a radiolabeled chimeric monoclonal antibody imaging agent being developed by GlyTherix for positron emission tomography (PET) imaging of tumors expressing glypican-1 (GPC-1). Miltuximab is a chimeric human IgG1 antibody engineered from the parent antibody MIL-38, which specifically targets GPC-1, a cell surface protein overexpressed in several solid tumors, including prostate and bladder cancers. By conjugating miltuximab with the positron-emitting radioisotope zirconium-89 (89Zr), the agent enables the non-invasive detection, localization, and quantification of GPC-1-positive lesions in vivo. This diagnostic tool is part of a theranostic strategy, intended to identify and select patients who may benefit from treatment with the therapeutic counterpart, 177Lu-miltuximab.
