**89Zr-trastuzumab** is an investigational HER2-targeted immuno-PET radiopharmaceutical comprising the anti-HER2 monoclonal antibody trastuzumab labeled with the positron-emitting radionuclide zirconium-89, generally through a chelator such as desferrioxamine. After intravenous administration, it binds HER2-expressing tumor cells and enables PET/CT visualization of HER2 expression across primary and metastatic lesions. It is being evaluated as a noninvasive molecular-imaging biomarker for patient selection, assessment of HER2 heterogeneity, and prediction of benefit from HER2-directed therapies, including antibody-drug conjugates, in HER2-positive breast cancer. It is an imaging tracer rather than a therapeutic radiopharmaceutical.
