90Y DOTATATE + 177Lu DOTATATE is a combination peptide receptor radionuclide therapy (PRRT) that utilizes two different radiopharmaceuticals to target somatostatin receptor-positive tumors, particularly neuroendocrine tumors (NETs). This tandem therapy combines the higher energy and longer range beta emissions of yttrium-90 (which is more effective for larger tumors) with the lower energy and shorter range emissions of lutetium-177 (which is more suitable for smaller tumors). The combination approach aims to optimize tumor response while minimizing toxicity to healthy tissues. Each radioisotope is conjugated to DOTATATE, a somatostatin analog that binds with high affinity to somatostatin receptors (particularly SST-R2) expressed on the surface of neuroendocrine tumor cells.
