90Y-NM600 is a targeted radionuclide therapy (TRT) consisting of the therapeutic beta-emitting radioisotope yttrium-90 (90Y) conjugated to NM600, a tumor-targeting alkylphosphocholine (APC) or phospholipid ether (PLE) analog. NM600 selectively accumulates in the plasma membranes of many cancer cell types by exploiting the overabundance of lipid rafts—sphingomyelin and cholesterol-rich membrane domains—relative to healthy cells. This mechanism allows for the delivery of high-energy ionizing radiation directly to primary and metastatic tumor sites, causing localized DNA damage and cell death while sparing normal tissues. 90Y-NM600 is being investigated as an 'in situ' vaccine or immunomodulator to transform cold tumors into hot tumors, potentially enhancing the efficacy of immune checkpoint inhibitors such as anti-PD-1 and anti-PD-L1 therapies. It is typically used in a theranostic approach alongside 86Y-NM600, which serves as a PET imaging agent for patient-specific dosimetry.
