99mTc-cFLFLF is a radiopharmaceutical imaging agent consisting of the synthetic chemotactic peptide cinnamoyl-F-(D)L-F-(D)L-F (cinnamoyl-Phe-D-Leu-Phe-D-Leu-Phe), which is conjugated to polyethylene glycol (PEG) and hydrazinonicotinamide (HYNIC), then labeled with technetium-99m. This compound specifically targets formyl peptide receptors (FPRs) on neutrophils, allowing for the detection of sites of infection and inflammation using SPECT/CT imaging. The mechanism involves high-affinity binding to FPRs expressed on activated neutrophils that accumulate at inflammatory foci. 99mTc-cFLFLF has demonstrated superior specificity and image quality compared to conventional bone or FDG PET tracers in preclinical models of acute osteomyelitis and has shown promise in early clinical evaluation for detecting both infectious and sterile inflammation. It is under development primarily as a diagnostic tool for early detection and therapeutic monitoring of infectious diseases such as osteomyelitis, pneumonia, and postsurgical inflammation[1][2][4].
