Technetium-99m dextran 500 is a radiopharmaceutical agent consisting of high molecular weight dextran (average molecular weight approximately 500,000 Daltons) labeled with the radioisotope technetium-99m. It is used primarily as a diagnostic imaging agent in nuclear medicine for lymphoscintigraphy and mapping of the lymphatic system, including sentinel lymph node detection. The mechanism involves intravenous or subcutaneous administration; the large size of the labeled macromolecule allows it to be retained within lymphatic vessels and nodes after local injection. The gamma emissions from technetium-99m enable external imaging using gamma cameras to visualize lymphatic drainage patterns and identify sentinel nodes for biopsy or surgical planning[1][5][6]. Dextrans are polysaccharides composed mainly of α-(1→6) linked glucose units with branching at α-(1→3) or α-(1→4). When radiolabeled with Tc-99m, they serve as inert carriers that do not undergo significant metabolism during imaging procedures.
