99mTc-EC-metronidazole is a diagnostic radiopharmaceutical composed of technetium-99m labeled ethylene dicysteine conjugated to metronidazole. It is designed for use with brain SPECT imaging to detect hypoxic but viable brain tissue—such as peri-infarct regions—after acute ischemic stroke. The presence and uptake of this compound in hypoxic tissue provides information that is valuable for prognostication of neurological recovery. The mechanism of imaging is based on the selective accumulation of metronidazole derivatives in hypoxic tissues, where metronidazole acts as a DNA synthesis inhibitor in the target tissue after being reduced in hypoxic conditions. The compound is mainly utilized for neurological indications, especially for acute ischemic stroke, and has been explored for cancer imaging.
