Drug intelligence / Profile preview
99mTc-HYNIC-exendin-4 (also known as [Lys40(Ahx-HYNIC-99mTc/EDDA)NH2]-exendin-4) is an investigational radiodiagnostic peptide tracer used for single-photon emission computed tomography (SPECT) or SPECT/CT imaging of the glucagon-like peptide-1 (GLP-1) receptor. The tracer consists of exendin-4, a GLP-1 receptor agonist peptide that is resistant to dipeptidyl peptidase IV (DPP-4) degradation, conjugated to a hydrazinonicotinamide (HYNIC) chelator via an aminohexanoic acid (Ahx) spacer at a lysine-40 residue, and radiolabeled with technetium-99m (99mTc) using ethylenediaminediacetic acid (EDDA) as a co-ligand. It is primarily developed for the preoperative localization of occult insulinomas (insulin-secreting pancreatic neuroendocrine tumors) and other conditions associated with endogenous hyperinsulinemic hypoglycemia (EHH), such as nesidioblastosis, particularly in patients where conventional imaging modalities (CT, MRI, ultrasound) are negative or inconclusive.
