A radiopharmaceutical composed of a small engineered protein (affibody) that specifically binds to human epidermal growth factor receptor type 2 (HER2), labeled with the gamma-emitting radionuclide technetium-99m (99mTc). The HER2 affibody is a non-immunoglobulin scaffold protein (~6.5–7 kDa) designed for high-affinity and selective binding to HER2 receptors overexpressed in certain cancers such as breast cancer. Upon intravenous administration, the radiolabeled affibody binds to HER2-expressing tumor cells and enables their detection by single-photon emission computed tomography (SPECT) imaging. This allows for non-invasive assessment of HER2 status in tumors and metastases to guide targeted therapy decisions[1][3][4][6][8]. Multiple variants exist based on different chelators or sequence modifications but share the same core mechanism.
