99mTc-MAG3-Pep-1 is a radiolabeled peptide, composed of the synthetic peptide "Pep-1" conjugated to mercaptoacetyltriglycine (MAG3), which is then labeled with technetium-99m (99mTc). While 99mTc-MAG3 alone is a well-established renal radiopharmaceutical primarily used for renal scintigraphy and assessment of renal function, the addition of a "Pep-1" moiety indicates this is a targeted imaging agent, where Pep-1 may serve as a vector or ligand for specific molecular targets (such as cancer biomarkers or receptors), enabling targeted single photon emission computed tomography (SPECT) imaging. This agent works through delivery of a gamma-emitting 99mTc isotope to sites of Pep-1 binding, allowing for noninvasive molecular imaging of tissues expressing the Pep-1 target. Mechanisms and specific indications depend on the "Pep-1" peptide’s target, but such radiopharmaceuticals are typically developed for molecular imaging in oncology, tissue targeting, or receptor mapping.
