99mTc-PSMA-T4 is a diagnostic radiopharmaceutical developed by the National Centre for Nuclear Research (Narodowe Centrum Badań Jądrowych - POLATOM) in Poland for the imaging of prostate cancer. It consists of a small-molecule peptide ligand, T4, which exhibits high affinity for the prostate-specific membrane antigen (PSMA), a protein significantly overexpressed on the surface of prostate cancer cells. The ligand is chelated to technetium-99m (99mTc), a gamma-emitting radioisotope widely used in nuclear medicine. Upon intravenous administration, the tracer binds to PSMA-positive tumor sites, allowing for their visualization and localization using Single Photon Emission Computed Tomography (SPECT) combined with Computed Tomography (CT). The agent is primarily intended for the staging of high-risk prostate cancer and the detection of recurrence in patients with biochemical failure.
