99mTc-RM26 is a radiopharmaceutical diagnostic agent composed of the synthetic bombesin analogue peptide RM26 labeled with the radionuclide technetium-99m (99mTc). RM26 is a high-affinity antagonist that specifically targets the **Gastrin-Releasing Peptide Receptor (GRPR)**, also known as the bombesin receptor subtype 2 (BB2). GRPR is significantly overexpressed in several human malignancies, most notably prostate and breast cancers, while showing low expression in normal tissues. By binding to these receptors, 99mTc-RM26 enables the visualization and localization of primary tumors and metastatic lesions using **Single-Photon Emission Computed Tomography (SPECT)**. As an antagonist, RM26 offers advantages over agonist-based tracers, including higher metabolic stability and a potentially safer profile due to the lack of receptor activation upon binding. The agent is primarily investigated for its utility in the molecular staging and management of prostate and breast cancer patients.
