A-ECO red blood cells are a cell therapy product consisting of group A red blood cells that have been enzymatically modified to remove the A-antigen from their surface, effectively converting them into group O (universal donor) cells. Developed by Velico Medical, this technology utilizes specific recombinant alpha-N-acetylgalactosaminidases (glycosidases) to cleave the terminal alpha-1,3-linked N-acetylgalactosamine residue that defines the A blood group. By converting the A-antigen into the H-antigen found on group O cells, the product aims to mitigate the risk of hemolytic transfusion reactions and address shortages in the supply of O-negative and O-positive universal donor blood. Clinical development has focused on evaluating the safety, immunogenicity, and post-transfusion survival of these converted cells in autologous and allogeneic settings.
