A20FMDV2 is a 20-amino-acid peptide (sequence: NAVPNLRGDLQVLAQKVART) derived from the VP1 coat protein of the foot-and-mouth disease virus (FMDV). It functions as a highly selective and high-affinity ligand for the epithelial-specific integrin αvβ6, a receptor that is significantly upregulated in various aggressive malignancies—including pancreatic ductal adenocarcinoma, cervical cancer, and non-small cell lung cancer—while remaining virtually absent in healthy adult tissues. Due to this specificity, A20FMDV2 is primarily developed as a targeting moiety for radiopharmaceuticals. When conjugated with chelators (such as DOTA or PCTA) and radiolabeled with isotopes like Copper-64 or Fluorine-18, it enables the non-invasive detection of αvβ6-positive tumors and metastatic lesions via positron emission tomography (PET). Modifications such as bi-terminal PEGylation are frequently employed to enhance its metabolic stability and optimize its pharmacokinetic profile for clinical use.
