A7-neocarzinostatin (A7-NCS) is an early-generation antibody-drug conjugate (ADC) designed for the targeted treatment of gastrointestinal malignancies, specifically colorectal, pancreatic, and gastric cancers. It consists of the murine monoclonal antibody A7, which targets a specific glycoprotein antigen expressed on these tumor cells, covalently linked to neocarzinostatin (NCS), a potent macromolecular enediyne antibiotic. NCS acts as a DNA-damaging agent, inducing strand breaks and inhibiting DNA synthesis. Developed and clinically evaluated primarily in Japan, A7-NCS was administered via intraarterial or intratumoral routes to maximize local concentration and minimize systemic toxicity. Clinical trials demonstrated its ability to localize specifically to tumor tissues and provide palliative benefits, such as tumor reduction and pain relief in patients with advanced colorectal cancer and liver metastases.
