**AAI101 + piperacillin** is an investigational combination therapy consisting of **AAI101**, a novel extended-spectrum beta-lactamase inhibitor, and **piperacillin**, a broad-spectrum antipseudomonal penicillin antibiotic. AAI101 is a penicillanic acid sulfone structurally related to tazobactam but distinguished by a specific methyl modification enhancing its cell penetration. It effectively inhibits class A, C, and some D beta-lactamases, including ESBLs (e.g., CTX-M and SHV types), which confer resistance to many beta-lactam antibiotics. By inhibiting these enzymes, AAI101 restores or enhances the antibacterial activity of piperacillin against extended-spectrum beta-lactamase-producing and some multidrug-resistant Gram-negative bacteria. This combination has been shown in preclinical studies to be active against piperacillin-resistant ESBL-producing Klebsiella pneumoniae and Escherichia coli, reducing the emergence of resistance when used at appropriate doses. The combination is under investigation and not yet approved for clinical use[1][2].
