Abatacept is a recombinant fusion protein composed of the extracellular domain of cytotoxic T-lymphocyte-associated antigen 4 (CTLA-4) fused to the Fc region of human immunoglobulin G1 (IgG1). It acts as a selective T-cell costimulation modulator by binding to CD80 (B7-1) and CD86 (B7-2) on antigen-presenting cells, thereby blocking their interaction with CD28 on T cells. This inhibition prevents the second signal required for full T-cell activation, leading to reduced proliferation and activity of T cells involved in autoimmune responses. Abatacept is primarily indicated for moderate-to-severe rheumatoid arthritis in adults who have not responded adequately to other therapies, polyarticular juvenile idiopathic arthritis in children aged 2 years and older, psoriatic arthritis in adults and children aged 2 years and older, and prevention of graft-versus-host disease following hematopoietic stem cell transplantation. It can be administered intravenously or subcutaneously[1][3][5][6].
