Abelacimab is a fully human monoclonal antibody that acts as a highly selective inhibitor of Factor XI and its activated form, Factor XIa. By binding to the catalytic domain of both the inactive zymogen (Factor XI) and activated Factor XIa, abelacimab locks them in an inactive conformation, preventing their procoagulant activity. This mechanism allows for effective anticoagulation while minimizing disruption to normal hemostasis. Abelacimab is administered via intravenous or subcutaneous injection and has demonstrated rapid onset and sustained inhibition of Factor XI activity for up to 30 days with monthly dosing. It is being developed primarily for the prevention of stroke and systemic embolism in patients with atrial fibrillation at moderate-to-high risk, as well as for venous thromboembolism prevention after surgery. Clinical trials have shown significant reductions in major bleeding compared to standard-of-care anticoagulants such as rivaroxaban[1][2][3][4][5][6][7].
