Overview

This is a combination regimen of four oral agents used in oncology, particularly for multiple myeloma and potentially other hematologic malignancies. **Abemaciclib** is a selective inhibitor of cyclin-dependent kinases 4 and 6 (CDK4/6), blocking cell cycle progression from G1 to S phase. It is primarily developed for breast cancer but may be explored in other cancers. **Dexamethasone** is a synthetic glucocorticoid with anti-inflammatory and immunosuppressant properties, commonly used as part of multiple myeloma regimens to reduce inflammation and tumor burden. **Ixazomib** is an oral proteasome inhibitor that disrupts protein degradation pathways, leading to apoptosis in malignant plasma cells; it is approved for use with lenalidomide/dexamethasone in relapsed/refractory multiple myeloma[8]. **Pomalidomide** is an immunomodulatory agent that enhances T-cell and natural killer cell activity while inhibiting angiogenesis; it’s approved for relapsed/refractory multiple myeloma[5][6]. The triplet combination of ixazomib, pomalidomide, and dexamethasone has demonstrated efficacy in lenalidomide-refractory or double-refractory multiple myeloma patients[1][3][5][6]. The addition of abemaciclib would be investigational; its mechanism complements the others by targeting cell cycle regulation.
