This is a hypothetical combination of four agents—abemaciclib, fulvestrant, palbociclib, and ribociclib—all of which are individually approved for the treatment of hormone receptor-positive (HR+), human epidermal growth factor receptor 2-negative (HER2-) advanced or metastatic breast cancer. Abemaciclib, palbociclib, and ribociclib are small molecule inhibitors targeting cyclin-dependent kinases 4 and 6 (CDK4/6), thereby blocking cell cycle progression from G1 to S phase in cancer cells. Fulvestrant is a selective estrogen receptor degrader (SERD) that binds to the estrogen receptor (ER), leading to its degradation and downregulation of ER signaling. While each CDK4/6 inhibitor has been studied in combination with fulvestrant or aromatase inhibitors for HR+/HER2- breast cancer—with demonstrated improvements in progression-free survival—the simultaneous use of all three CDK4/6 inhibitors together with fulvestrant is not an established or clinically studied regimen[3][5]. There are no clinical data supporting the safety or efficacy of combining all three CDK4/6 inhibitors plus fulvestrant.
