Abemaciclib is a **selective inhibitor of cyclin-dependent kinase 4 and 6 (CDK4/6)**. In the adjuvant setting, it is combined with **standard adjuvant endocrine therapy** (ET, typically tamoxifen or an aromatase inhibitor) for the treatment of **hormone receptor-positive (HR+), human epidermal growth factor receptor 2-negative (HER2-), node-positive, early breast cancer at high risk of recurrence**. The combination is used for disease settings with higher recurrence risk, as defined in pivotal monarchE clinical trials. Mechanistically, abemaciclib blocks cell cycle progression at the G1 to S phase, thereby inhibiting proliferation of malignant cells. Endocrine therapy acts by inhibiting estrogen signaling, either via direct receptor antagonism (tamoxifen) or by blocking estrogen synthesis (aromatase inhibitors). The combination has demonstrated clinically meaningful improvements in invasive disease-free survival compared to endocrine therapy alone, and is FDA approved for this adjuvant indication[2][3][4][5][6][8].
