Abiraterone acetate + dexamethasone is a combination therapy used in the treatment of metastatic castration-resistant prostate cancer (mCRPC) and has also been studied as an alternative to the standard combination of abiraterone acetate plus prednisone. **Abiraterone acetate** is a prodrug of abiraterone, a selective and irreversible inhibitor of cytochrome P450 17A1 (CYP17A1), an enzyme critical for androgen biosynthesis, leading to reduced androgen production. This decrease in androgen levels inhibits the growth of androgen-dependent prostate cancer cells. **Dexamethasone** is a potent synthetic glucocorticoid that, when combined with abiraterone, mitigates the mineralocorticoid excess caused by CYP17A1 inhibition and may also provide additional anti-cancer effects, possibly through antiangiogenic and immunosuppressive mechanisms. The combination is used particularly in mCRPC patients who progress on abiraterone plus prednisone, as switching steroids can restore response in some cases.
