AC-OLE-02 is an investigational liquid formulation of tricaprilin, a medium-chain triglyceride (MCT) composed of three eight-carbon fatty acids (caprylic acid) esterified to a glycerol backbone. Developed by Cerecin, it is intended for the treatment of Alzheimer's disease. The drug functions as a ketogenic precursor; following oral administration, it is metabolized in the liver to produce ketone bodies, specifically beta-hydroxybutyrate. These ketone bodies serve as an alternative energy source for neurons in the brains of Alzheimer's patients, which typically exhibit impaired glucose metabolism (hypometabolism). AC-OLE-02 was specifically designed to improve the pharmacokinetic profile and gastrointestinal tolerability compared to earlier formulations like AC-1202. It was evaluated in a Phase 1 clinical trial (NCT04183400) to assess its safety, tolerability, and pharmacokinetics in healthy participants.
