Acamprosate is a small molecule drug used to help maintain abstinence in people with alcohol dependence who have already stopped drinking. It is not effective for treating withdrawal symptoms or for individuals who have not ceased alcohol use. Acamprosate acts primarily by modulating and normalizing the balance of neurotransmitters in the brain that are disrupted by chronic alcohol consumption. Its mechanism of action is thought to involve interactions with glutamate (specifically NMDA receptors) and GABA neurotransmitter systems, reducing glutamatergic hyperactivity associated with protracted withdrawal and possibly enhancing GABAergic activity. Structurally, it is an analog of gamma-aminobutyric acid (GABA) and taurine. The drug does not undergo significant metabolism and is excreted mainly via the kidneys[1][3][5][7][10].
