Overview

**Acamprosate + baclofen** is Pharnext's low-dose, orally administered repurposed-drug combination developed as PXT864 and PXT00864 for mild Alzheimer's disease. Baclofen is a gamma-aminobutyric acid type B receptor agonist, while acamprosate has incompletely resolved pharmacology and is thought to modulate glutamatergic neurotransmission. Pharnext proposed that combining the agents could restore disrupted inhibitory GABAergic and glycinergic versus excitatory glutamatergic signaling associated with amyloid-beta oligomer toxicity. The PLEODIAL-I and PLEODIAL-II studies evaluated the combination in mild Alzheimer's disease; Pharnext stopped its Alzheimer's disease development program in February 2023 for financial and prioritization reasons.
