Acazicolcept is a first-in-class recombinant fusion protein designed as a dual inhibitor of the CD28 and inducible T-cell co-stimulator (ICOS) costimulatory pathways. It is an Fc fusion protein composed of a human inducible T cell costimulator ligand variant immunoglobulin domain fused to a human IgG1 Fc fragment. By simultaneously blocking both the CD28 and ICOS pathways, acazicolcept aims to suppress aberrant T cell activation implicated in autoimmune and inflammatory diseases. The drug is being developed primarily for systemic lupus erythematosus (SLE), with additional preclinical investigation in other autoimmune conditions such as arthritis, type 1 diabetes mellitus, uveitis, Sjögren’s syndrome, inflammatory bowel disease, multiple sclerosis, and graft-versus-host disease[1][2][3][6][10].
