Aceglutamide is an acetylated derivative of the amino acid L-glutamine that functions as a neuroprotective agent, psychostimulant, and mucosal protective agent. It is primarily utilized in the treatment of cerebrovascular and neurological conditions, including ischemic stroke, traumatic brain injury, and cognitive impairment. Aceglutamide effectively crosses the blood-brain barrier, where it acts as a precursor to the neurotransmitters glutamate and GABA. Its neuroprotective mechanism involves the activation of the **Akt/Bcl-2 signaling pathway** and the inhibition of the pro-apoptotic factor **TRAF1**. Additionally, it exhibits potent antioxidant properties by enhancing the **Nrf2**, glutathione (GSH), and thioredoxin (Trx) systems, and exerts anti-inflammatory effects by suppressing **NF-κB** translocation and the production of proinflammatory cytokines. In clinical practice, particularly in China, it is frequently administered as a component of **Guhong Injection**, a combination therapy with safflower extract (*Carthamus tinctorius*). Its aluminum salt, aceglutamide aluminum, is also used orally to treat peptic ulcers by providing mucosal protection.
