Pain Relief PM is an over-the-counter (OTC) fixed-dose combination medication containing the non-opioid analgesic acetaminophen (paracetamol) and the first-generation antihistamine diphenhydramine hydrochloride. Acetaminophen acts primarily in the central nervous system to inhibit prostaglandin synthesis via cyclooxygenase (COX-1 and COX-2) inhibition, thereby relieving minor aches and pains. Diphenhydramine hydrochloride acts as an inverse agonist at central histamine H1 receptors, crossing the blood-brain barrier to induce drowsiness and promote sleep. This combination is indicated for the temporary relief of occasional headaches and minor aches and pains accompanied by sleeplessness.
