Acetaminophen + NSAIDs (referred to as 'PHARMA' in the EXERPHARMA trial) is a combination analgesic regimen consisting of acetaminophen (paracetamol) and non-steroidal anti-inflammatory drugs (NSAIDs) used as an optimized pharmacological pain management strategy. This regimen is primarily indicated for the relief of mild to moderate pain and inflammation, such as that associated with knee osteoarthritis. The mechanism of action involves the inhibition of cyclooxygenase (COX) enzymes by the NSAID component, which reduces the synthesis of prostaglandins responsible for inflammation and pain signaling. Acetaminophen contributes to analgesia by modulating central and peripheral pain pathways, although its exact molecular target remains a subject of ongoing research. In the context of the EXERPHARMA trial conducted by the University of Southern Denmark, this pharmacological intervention was compared against neuromuscular exercise (NEMEX) to evaluate its impact on knee-joint loads in patients with medial tibiofemoral knee osteoarthritis.
