Acetyl-DL-leucine is a racemic (DL) mixture of the acetylated derivative of the amino acid leucine. It has been used since the 1950s, particularly in France, for symptomatic treatment of acute vestibular vertigo and dizziness. The compound is also studied for its neuroprotective properties and potential benefits in neurological disorders, including Niemann-Pick disease type C and cerebellar ataxia. Its mechanism of action is not fully understood but may involve restoration of membrane potential in injured vestibular neurons via interaction with membrane phospholipids, inhibition of mTORC1 to promote autophagy, enhancement of glucose utilization in brain cells, and balancing pH by facilitating lactate removal from neurons[3][4][5]. It does not bind or block LAT1 transporters[4]. Acetyl-DL-leucine is also used as a cognitive enhancer in dietary supplements and has applications in veterinary medicine[3].
